What are some good eating and workout plans that have worked well for any of you? I'm not currently following any plan. I do eat pretty healthy and try to hit the gym atleast 3 times a week and I've just been gaining weight.

I used Sparkpeople to track my calories and workouts up until recently. It's a really good site. You put in your information and you can have them make meal/exercise plans for you or they can give you ranges for calories to stick between and you can add your own food. Loseit and Myfitnesspal are good too.
If you don't want to track online, I definitely suggest at least making a notebook where you can keep track of your meals, exercises, measurements, weight, etc. so that you can see what works for you and what doesn't.
If you eat healthy already, maybe you should check the serving sizes and make sure you're not having too much of anything, even the healthy things.
